This policy explains how we collect, use, disclose, protect, retain, and manage personal information and personal health information in connection with our pharmacy, compounding, travel clinic, vaccination, injection, and website services.
This summary is provided for readability. The full policy below explains how Sirocco Pharmacy handles personal information and personal health information in more detail.
This policy applies to personal information and personal health information collected, used, or disclosed by Sirocco Pharmacy through in-person pharmacy services, prescription dispensing, refills, transfers, medication reviews, compounding services, travel clinic consultations, vaccinations and injections, minor illness or clinical services, senior care support, phone, email, website and online forms, and appointment booking or third-party booking platforms used by the pharmacy.
This policy does not govern third-party websites, platforms, or services that Sirocco Pharmacy does not control. Where third-party tools are used, patients should review those third-party privacy policies.
Sirocco Pharmacy is committed to handling personal information and personal health information in accordance with applicable Alberta privacy laws and pharmacy professional obligations. Depending on the service and context, these may include Alberta's Health Information Act, Alberta's Personal Information Protection Act, Alberta College of Pharmacy standards and ethical expectations regarding confidentiality and patient information, and other applicable legal or professional obligations.
Where federal privacy legislation applies to commercial activities, Sirocco Pharmacy also considers its obligations under the Personal Information Protection and Electronic Documents Act (PIPEDA). Where authorized under Alberta’s Health Information Act, Sirocco Pharmacy may access and contribute to Alberta Netcare, the provincial electronic health record, as part of providing patient care.
Where Sirocco Pharmacy acts as a custodian or affiliate handling health information, health information is handled with the confidentiality and safeguards expected for patient health records. Where private-sector personal information is collected outside the health information context, private-sector privacy requirements may apply.
The information collected depends on the service requested and what is reasonably required to provide pharmacy care, manage records, communicate with patients, process claims, and meet legal or professional requirements.
Sirocco Pharmacy may disclose personal information or personal health information only with consent or when permitted or required by applicable law or professional obligations. We do not sell patient personal health information.
Consent may be express or implied depending on the service, the information involved, and the circumstances. Patients may withdraw consent, subject to legal, professional, safety, recordkeeping, or contractual limits. Withdrawing consent may limit Sirocco Pharmacy's ability to provide certain services.
Patients may express wishes about how their health information is disclosed. Sirocco Pharmacy will consider expressed wishes as required by applicable law and will take reasonable steps to respect patient privacy while continuing to provide safe pharmacy care.
Our website may collect basic technical information such as IP address, browser type, device type, pages visited, referral source, approximate location, and usage information. Cookies may be used for site functionality, security, analytics, and user experience.
Our website currently uses Google tools, including Google Analytics for aggregated site-usage statistics and Google reCAPTCHA to protect online forms from abuse; information collected by these tools is subject to Google’s privacy policy. Online appointment booking is provided through an external booking platform that operates under its own privacy policy. Patients can limit or delete cookies through their browser settings.
Online forms and email may not be appropriate for urgent or highly sensitive health information. Patients should call the pharmacy for urgent pharmacy questions. Email and internet transmissions may carry security risks. Sirocco Pharmacy takes reasonable steps to protect information but cannot guarantee the security of information sent over public networks.
Sirocco Pharmacy may use third-party platforms for appointment booking, prescription refill requests, payment processing, website hosting, analytics, email, pharmacy systems, or operational support. For example, appointment booking links on the website may direct patients to a third-party booking platform such as BookMyShot or another service used by the pharmacy.
Third-party providers may process information on Sirocco Pharmacy's behalf or under their own privacy terms. Patients should review third-party privacy policies before submitting information through external websites or tools.
Sirocco Pharmacy uses safeguards appropriate to the sensitivity of personal information and personal health information. These safeguards may include administrative, technical, and physical measures designed to protect records throughout their lifecycle.
Sirocco Pharmacy retains personal information and health information for as long as necessary to provide services, meet legal and professional recordkeeping obligations, resolve disputes, and comply with regulatory requirements. When information is no longer required, it is securely destroyed, de-identified, or otherwise handled according to applicable requirements.
Patients may request access to their own personal information or personal health information and may request correction of information they believe is inaccurate or incomplete, subject to applicable legal limits.
If Sirocco Pharmacy becomes aware of a privacy breach involving personal information or personal health information, it will assess the incident, take steps to contain and reduce harm, and notify affected individuals and/or regulators where required by applicable law.
For HIA-related health information breaches, notification obligations may include affected individuals, the Office of the Information and Privacy Commissioner of Alberta, and the Minister of Health where required. For PIPA-related personal information breaches, notification to the Commissioner may be required where there is a real risk of significant harm.
Parents, guardians, caregivers, or authorized representatives may provide or receive information where permitted by law and where appropriate authorization exists. Sirocco Pharmacy may need to verify authority before sharing information and respects privacy and consent requirements for minors and capable patients.
Privacy questions, concerns, access requests, or correction requests may be directed to the Sirocco Pharmacy Privacy Officer.
